EAD/AP documents for derivative

Hello

Can you guys please guide me on what all documents we need to submit for EAD/AP application for the derivative applicant?

1. Primary already filed 485 in 2012 since has EAD/AP, but maintaining H1B Status
2. In Oct 2020, the lawyer just filed 485 for Spouse, her H4/H4EAD is pending since Mar 2020 at CSC

This is what I think should be sent to get EAD/AP

1. 485 receipts copy
2. Passport Biograph page
3. 2 + 2 color photos
4. Form I131 and I765
5. Latest I94
6. Pending or approved I797 receipt of the current status

Do we need to send a copy of the primary I-140?
