RFE - Affidavit from parents

Hi, I have an RFE about birth certificate. My birth certificate is not issued on the same day as on my birth day or location. I got it issued recently so, I need an affidavit from my parents stating I am their son and other birth detail such as hospital, address etc. Can anyone have any sample of such affidavit letter or any web link? I would really appreciate the help. Thanks.
