    Birth Certificate Affidavit language

    My birth certificate has following deficiencies :

    1) Late registration (was registered 2 years after birth)

    2) Has only my first name and missing my mothers name.

    I have asked my parents to create 2 affidavits with all the information. I found affidavit sample in one of the threads on this forum.

    My question is usually in end of affidavit people mention

    I am submitting affidavit for the late registration
    or
    I am submitting affidavit as official birth record is incomplete.

    In my case both the above conditions are true.

    Will following lines just before my parents signature are fine or not :

    To my knowledge XYZ’s birth was not recorded by any government agency at the time of birth as it was not mandatory at that time.

    This affidavit is being submitted because the official record of birth is incomplete concerning XYZ.


    I will appreciate is fome one can anwser my query.

    As long as the affidavits state under oath that they are aware of your birth at xyz place on aa.bb.nnnn date, it should suffice.
    Might also simple add a note that the father's name was sufficient to register birth at the time of registration and that
    the norm/custom in your culture is a simple single name for the child and that no family name was included / required then.

    So in short as long as affidavits are under oath (which they are) there is no need to even mention the below 2 items specifically
