Numerous RFEs are reported in trackitt as well as in our forum. Spec has posted a graphical representation with the recipients' PD as reported.
We are not sure whether this is a system glitch or not, You may check those discussions at EB2-3 Predictions thread.

My lawyers received the actual notices this morning. They told me that basically my adjustment case has been pending for such a long time that USCIS would like to confirm that I am still eligible to adjust based on my company's I-140. They said they'll submit the required evidence.

Yes & Yes. When I asked my lawyers about that, they said "USCIS’ RFE for your spouse’s case is incorrect in saying that your spouse is a principal applicant of an employment-based adjustment application. We are going to say that your spouse is adjusting as a dependent of you, who has also received an identical RFE and responding to the RFE with evidence."
