Birth Certificate Related

I have a similar problem. There is a "ben" attached to my mother's name. What you can do is explicitly specify in the affidavit that:

Lakshmi and Laxmi are different ways to spell the name from your native language of the region into English. This should not be considered name change. The following names refer to one and same person:
list down all full names like lastname lakshmi middle, lastname laxmi middle etc

Next write down this affidavit is being provided to clarify the naming translation and is being provided as a proof of your birth and parentage.

Remember the affidiavits are first person from the deponent's point of view. so the language would be as if your mother is saying it.

Thanks a lot for the response. That helps. I will try to type out some matter for an affidavit and see if you and other gurus here can review it and suggest corrections if necessary.

I have medical exam due tomorrow. The form I-693 asks for A#. Does anyone know is it the same # given on I-140 approval notice or is it different ?
Sorry for the off topic question when everybody is tense.. I googled it and found answers like it is I-94 # or it is red # printed on Visa (H1) etc..

Use the one on your I-140 approval notice, that would be my advise.

Sorry to hear about your wifes stamping issues. Although you may have to contact a lawyer...

Your wife has to be in US for all the documents to be ready like doctors certificate etc.

Without knowing much about your wifes work situation - I think this is what you should do - Try to get the passport back from the consulate. Have your wife apply for H4 and let her come here ASAP. File for 485. Get EAD in 3 months and start working.

Contact a good Attorney!

Congratulations!

Schedule appointment as early as possible, irrespective of due date. Depending on her situation at the time of appointment Civil Surgeon will decide what to test or give vaccines.

Pregnant women are exempt from vaccines and so would be women nursing. I think your civil surgeon will tell you the same. It happened with my friends wife. She had to get hers done just before they received GC finally.

Just out of curiosity, this question came to my mind today and this is not related to immigration itself.

I came here on F1 and did my Masters in Public Health (MPH) from a US University. Meanwhile, while doing masters, I gave my US Medical Licensing Exams, obtained a residency in Internal Medicine and converted my visa in to H1B. I finished my MD in Internal Medicine from a US Hospital.

My question is if in case, the Congress passes a legislation to give green cards to international students with STEM (Science, Technology, Engineering, Math) degree, would my MPH or MD qualify under any of the four categories?

Any input would be appreciated.

There is only one way to be sure. See if the CIP code on your I-20 is listed in the STEM list on the ICE.gov website.

They are overcharging you my friend. We did our medical tests last month and it was $ 160 per person. The vaccines etc are over and above that.

Getting them through your PCP is ideal, even if you have to delay your appointment with the immigration doc.
You have plenty of time.

The affidavits are likely to take the most time, depending on where they are being done and sent from.

Only purpose of bank statement is to show you and wife holding joint account, to prove marriage. You can alternatively provide lease agreement of house with you both on it, or title of home with both names on it, if you have purchased a home. Of course marriage certificate.

I have only seen this kind of detailed query for Marriage proof when wife applies seperately. If you both apply together, and you have marriage certificate, and tax returns of at least one year show you both as joint return, there should not be any query.

Just got my medical done for me and my wife. It costed me $640 for both. Need to go back on saturday for blood work results and collect the envolpe.
My employer has fedex my employer verification document
Started to write forms.......i have few questions..need to check that
Need to take xerox of documents

Hopefully i should get all this done by this weekend and mail my package to attornety

I remember you mentioned you has done this for your better half. Can you post the wording of the affidavit, obviously removing details? I would like to get this done as soon as possible to avoid a near certain RFE. Although my PD is 1st Feb unlike many others with PD before me I am confident it should be next month :-).

You don't need to worry about getting you medical test in anticipation of getting current. You should however, as you are look into what vaccinations etc you need to get and take care of that through your PCP.
If you have all vaccinations etc documented, it will save you a lot of money as well as expedite the process. It will then take only 2-3 days.

Hello folks,

I don't have any record for Vaccination. Which vaccinations will I require for I 485 ?

I found online that MMR , TdaP are required one if doc find no immune in blood work. Besides these any other required ?

Also any doctor in NJ ( Northern or Central ) does it for cheap. I called two doc and they gave me 300 Per person for MMR , Blood work for Syphilis and office visit. If I need blood work for immune record or any other vaccination it may be 100 - 300 extra depends on how many I need. So min range 300 max range 600 they gave me.

Looks like too high. I called my insurance and they do cover immunization as preventive care , my PCP is also does that. I just need to know which vaccinations we can get from PCP and have them ready for this civil surgeon so I don't need any from him.

This is interesting. Did your lawyer actually issue this Non-Availability Certificate or am I mistaken? I was under the impression that it must be issued by the local authorities of the city/village where one was born. One of my friends who is in line to get current (several years from now) does not have an official BC and her family in India is trying to get a "non-availability" certificate from the local officials. It would be easier for her if a lawyer in US could issue this certificate. But I remember reading somewhere that it must be from local officials. Let me know how it works out for you. All the best!

It would be easier for her if a lawyer in US could issue this certificate. But I remember reading somewhere that it must be from local officials. Let me know how it works out for you. All the best!

No Lawyer did not issue this Non-Availablity Certificate. This is a template which can be used and signed by local authorities.

Called my attorney office to check on Affidavits for birth as we have our Birthdate registered last year. My attorney says if there is a secondary proof of birth( Tenth Marks Memo, PAN Card, Indian Driving licence) no need of affidavits. One less thing to worry.

I have my Birth Certificate with me, but the Registration Date on it is showing 2010 date. The Issue Date is also 2010 date. Would this be any problem? Do I have to get Affidavits from Parents or relatives? Please advise.

My wife has similar issue. I asked lawyer, he told me it's nothing to worry about, and he asked to provide high school leaving certificate as backup.

Per him, in fact, affidavit is not needed, if have secondary evidence like above. I am just preparing affidavits in advance to keep just in worst case, but lawyer is not asking or going to submit them.

My wife has the same issue. We tried to get it fixed on the BC (as the passport has correct detail) but the local authorities said it is not possible for them to say the person was born in City B when it is the City A where the person's birth was registered and that is issuing the certificate. I was really concerned about this and repeatedly asked my lawyers if we need to send affidavits or try and get passport updated to match the BC. They basically told me to not worry too much about the inconsistency and if at all there is an RFE, they will explain the issue and say that we tried to fix it but could not do it. I'm not totally satisfied with this. But I trust my lawyers as they have done such a great job (knock on wood) until now.
