This is what I did
1) Find a doctor using the link

I found the doctor using the above link and called them first. They knew exactly what needs to be done and I asked them whether they will provide a digital scan of all the pages before sealing it. The doctor office obliged with my request.

Downloaded the I-693 from the above link and did not use the one at the doctor's office. To be safe, its always better to use the latest version. Then I filled two applications one for me and wife. I filled as much as I can - the two pages. I didn't sign and date it and left it blank. One nice thing about filling it on the pdf, the header with name and A-number are pre-filled based on what you type in the first page. Then took the printouts and handed it at the front desk.
3) I usually go to Walgreens for my shots and hence went to Walgreens and asked for a report of all my previous vaccines shots and they provided a nice report. I also provided this at the front desk.
4) They went over my immunization record and asked me to take the missing ones and also gave me the list of blood tests to be taken. Usually, they partner with a lab and I called up the lab and scheduled the tests.
5) After, giving the blood test, it took almost a week for the doctor office to receive the results which was negative as its for STDs ;-) After that, scheduled an appointment with the doctor and for the actual appointment. This is my second medicals and every doctor has a different way of doing the physical exams. So don't be surprised if the doc asks you to pull down your pants :-)
6) After two days, I got a call saying my report is ready. I went to the office again and both of us signed and dated the second page. Then I asked them to email me the scanned copy before sealing it. I forwarded it to my lawyer and she OKed it. I'm lucky to have a lawyer who makes sure everything is perfect. I mailed the sealed I-693 to my lawyer instead of USCIS. The lawyer would have received a copy of the RFE as well. So the lawyer fedex the both forms along with the pink RFE document.
Anyways, after two days after my lawyer mailed the package, my case status was updated acknowledging that the USCIS received my I-693.
