Based on the information I have, for our age category we need the
MMR,
TDAP and
Varicella titers. If you have these within your blood, you dont need a vaccine. If not, then you need to take the vaccine. I am sure abt these 3, not sure if more are required recently. Maybe someone who has done their immigration medical with the civil surgeon recently mention if more are needed.

Particularly, look at the example : "For example, if the applicant is 35 years of age, then he or she is required to receive tetanus and diphtheria toxoids vaccine (Td) or diphtheria and tetanus toxoids and acellular pertussis antigen vaccine (Tdap), MMR, and varicella (if no history of varicella disease)."
Also -
"The civil surgeon may refer the applicant to another health-care provider to have the required vaccinations administered. In such a case, the civil surgeon must not complete the form until the applicant returns with a written record from the referral health-care provider that notes the vaccines administered and the dates of administration."
