Can you please answer my questions below? - The link provided (
http://india.gov.in/howdo/index.php) does not seem to provide info on corrections to certificates.
1. What documents are required to correct the spelling errors in the bc?
Depends on the local authority reissuing the certificate. A photocopy of the original is needed. If the correct spelling names appear in the Passport - first and last page, school leaving certificate etc. In my case, a photocopy of original and the first and last page of passport was enough as it showed clear connection between the original birth certificate and the passport - except for name everything else was same.
2. Should we go to any specific govt. authority who has jurisdication over the place of birth to get the bc corrected, in India? The reason being, I was born in a city different and far away from where my parents reside now and I am not sure how they can get it from my place of birth.
Yes you will most likely have to do that as there is no centralized system or database. Mostly likely the local municipal office. You don't need to send your parents. Anyone in that town you know will work who has the photocopies and can establish connection. In my case I sent scanned copies to my uncle and he printed them out.
3. If jurisdiction does not matter, does a general statement from my parents (notarized by any Notary Public) suffice?
If you are making an affidavit then yes.
4. Is there a specific template used for spelling correction? If so, can you please provide it as well? - Nishant, I looked at the templates you provided earlier in the forum but I am not sure, if they can be used for spelling corrections as well. Can you please throw some light?
When you make the affidavit you have to mention the reason for it. In your case the discrepancy in the spelling of your name etc. Use Nishant's template - it captures the important details you will need in the affidavit. But it does not need to be worded exactly. There is no format here. It is just a template. Consult an attorney in US for best advice.
