Vaccination Letter Format

Guys - This is my first post. This forum is really helpful to everyone and got lot of experts and gurus who really give valuable information when someone is needed.

I am one of those poor souls who recently got really high hopes that i will file my I485 in another 10-12 months ...So, i started collecting all the docs needed. I can get the vaccination list that had given to me during my childhood back from my home town.

Is there any specific format or template that i should follow to get that letter?
I know it should be on the letter head of the clinic/hospital with seal and sign ...
Do they have to list specifically the dates of the vaccination OR just the vaccination details ?...

Really appreciate your help on this ...

I don't recall providing a vaccination list when I did my medical and I am not sure that you need it. I think they do a blood test to determine if you need to get inoculated or not.
