I-131 Rejected - Incorrect form -Please help!

Longtime lurker. first time poster here!

I recently submitted I-131 (AP) and I-765 (EAD) for renewal but I incorrectly filed an outdated form I-131. As a result, my I-131 was rejected with the following reason:
"The application/petition was filed on an outdated version of this form. Please resubmit your application/petition on the current version of this form with appropriate fees to the address listed on the bottom of the page"

I am planning to resubmit but have the following questions:
1. Do I need to resubmit I-765 (EAD) form as well? Note that my I-765 application (first submission) has not been returned ?
2. Do I mention this rejection (in the cover letter) in my resubmitted material ?
