EAD and AP card replacement

Hi All,
Me and my wife applied for EAD/AP renewal in Feb 2013. I got my renewed EAD/AP combo card in March.
My wife's got lost in the mail. What is process for applying for a replacement card. And is there any fees
for replacing the combo card. I couldn't find any instructions one the process for the renewal.
Any pointers on how to go about it would be very helpful.

My card was lost, you need to re-apply for card. there is no duplicate card
