I last did my medicals (I-693) in year 2012. I need to do medicals again as a result of filing I485-Supp J. While searching using "USCIS Find a Doctor Tool (https://my.uscis.gov/findadoctor)", I could not locate the same doctor. He is not listed in the USCIS tool. Google says the 2012 doctor is still practicing with a different clinic. Looking practically, it would be easier to go with the old one. I may have to do medicals one more time before everything is said and done (late 2009 PD).

I understand that Civil Surgeons can be added or removed (revoked) from USCIS list. Should I go with a new doctor listed in USCIS tool or is it allowed to go with the old one?

Update: The doctor who did medicals in Jan 2012 is now deceased. That's why I could not find him in the USCIS system. There is a new doctor in his clinic. The cost is $345.

Personal experience: I go to the Concentra Urgent Care facility - have done 3 I-693s so far with them :-). They always have a Civil Surgeon in staff as they do a lot of immigration paperwork. I recommend them - very straight forward process although they are a little more expensive - something like ~500 / person IIRC.
