Gurus/Friends,
I need some information not relating to this thread. Please let me know tips/tricks on filing FOIA for getting PERM and I-140 related documents. Thanks in advance.

It's very simple, you just need to fill up form "G693" I believe, scan it send it as an email.
I did notarize my signature in the same form, as I was sending it via email. You will get reference no# via mail. (for me it took 5/6 weeks - depends on the load) I moved to a different address and sent another email with the reference no#. They sent out a confirmation mail, and I received the documents in a cd.

The correct form is G639. http://www.uscis.gov/files/form/g-639.pdf

Do you know what is "Petition or Claim Receipt #" on the form? Also is Section 3, 4 and 5 all required? I am sorry but I am kinda confused.

Sorry, it is form "G639" not "G693" section 3,4,5 are required at least I filled in those. I did not fill in "Petition or Claim Receipt #" but Alien #
